This volume is the eighth in the Research Advances Series and the fifth published by Plenum Press. The purpose of the series is to review new work in rapidly changing fields. We do not expect reviews to cover the whole field of work on alcoholism and addiction. Nor do we expect that they will be like annual reviews covering all work in a delimited field. Our reviews are designed to explore only the most exciting parts of the total field and to focus on conclusions that can be made about them. The series publishes one volume each year. Volume 8 is an omnibus rather than a theme volume in that a wide range of topics is covered, including research on alcohol, opiates, and tobacco. As usual, the greater emphasis is on alcohol research, reflecting the importance of the problem and the volume of work to be reviewed. With Volume 8 come some changes in the Editorial Board. It will be the last volume in the series for Robert E. Popham who has resigned from the Board. He has been with the series since its inception and has contributed a great deal to its development. The members of the Board are grateful for his help. We are adding two new members: Dr. Howard Cappell, whose field is experimental psychology, and Dr. Edward M. Sellers, in clinical medicine and pharmacology.

Each city was provided with training & on site technical assistance to help officers identify & analyze specific neighborhood drug problems. For example, officers were assisted in determining all relevant factors of drug sales in a certain location; what type of drugs were sold, during what specific time, & by whom. The project revealed underlying social & economic conditions contributing to drug problems.

Many of the questions related to the treatment of drug addiction originate from the confusion that still characterizes this issue, even though biology is irreversibly changing our outlook on the physiology and psychopathology of the mind. In fact, prejudices stemming from the archaic concept of mind-body dualism are so difficult to eradicate that even a skilled psychiatrist may find it hard to distinguish a psychiatric symptom from a socially transgressive behavior. The dilemma becomes even more salient for drug addiction, since many of the substances which induce abuse and dependence are illegal, and their production, trade and use are forbidden by law. If the use of heroin is connoted as a crime, it may become controversial to recognize its chronic sequelae as a disease. Hence, withdrawal symptoms may alternatively be attributed to drug effects, or labelled as immoral attitudes arising from a vicious personality. However, the physician's judgement, which has to be merely instrumental at improving the patient's quality of life, should never be influenced by such complex setting. In fact, the only concern of the physician must be that of ascertaining the causes of symptoms and of removing them: if symptoms are induced by the toxic effect of a drug, he has to use the most effective antidote, and then proceed towards a detoxification; if they are due to withdrawal, a substitutive substance should be given initially. In the case of narcotics, substitutive therapy on a maintenance basis may become mandatory.

Special attention is paid to the innovative RhMAT (Rheumatoid Arthritis Medication Assessment Tool), in addition to descriptions of the pharmacological management of RA, pharmacoeconomic and pharmacovigilance considerations, the benefits of seamless care, and case presentations.Rheumatoid arthritis is one of the most common forms of inflammatory arthritis in the world, with a prevalence of 0.5 to 1%. While no cure has yet been established, modern biotechnology has enabled highly effective management, if treatment begins early. However, cost and side effects, such as immune suppression, continue to present barriers, and monitoring of patients is pivotal to safe and effective disease management. Both hospital and community pharmacists are involved in RA patient management, and have responsibilities to this patient population. Identifying pharmaceutical care issues and ensuring that the patients are managed in accordance to best evidence-based medicine are paramount. Best care is delivered when pharmacists effectively communicate with each other, the prescribers and the multidisciplinary team members involved in the care of the patient. This book aims to tackle the various aspects of the management of RA patients across all the settings.

During the last few centuries, and particularly in recent decades, problems result ing from the excessive use of drugs and alcohol have spread virtually as an epidemic to every country in the world and to almost every community. Abuse of alcohol and drugs is related to numerous other health problems, such as the spread of acquired immune deficiency syndrome (AIDS) on all continents. Health and social services today cannot afford to ignore this crisis since it affects all levels of society and manifests itself in diverse health and social problems. In recent years, the World Health Organization has received numerous requests for training material for physicians in this field. This manual therefore meets an urgent need. The availability of new data and the dearth of suitable textbooks have made its preparation mandatory. The influence of sociocultural factors on drug dependence and alcohol related problems-on their cause, development, and consequences as well as on their treatment and prevention-has been taken into account in the preparation of this manual in order to ensure that its usefulness is not limited to one country or region. It has been prepared primarily for the teaching of physicians and medical students, although much of it is relevant to the training of nurses, midwives, health educators, primary-care workers, medical social workers, counselors, and psychologists. In fact, suggestions have been included for adapting the manual for use in the training of such varied groups of students.

The rate of acute drug-related mortality, or overdose deaths, among prisoners in the immediate post-release period is unacceptably high. Such incidents result from many factors, including decreased tolerance after a period of relative abstinence during imprisonment and the concurrent use of multiple drugs which, with every additional illicit drug consumed in combination with opioids, nearly doubles the risk of death from opioids. Other important factors are the lack of pre-release counselling, post-release follow-up and failure to identify those at risk. Substance dependence is a chronic disorder with high relapse rates and often requires long-term continuous treatment. There is good evidence from trials and cohort studies that opioid substitution treatment reduces the risk of overdose among opioid users. This report identifies the main areas that need to be improved in order to decrease the risk of death. Linking prison health and public health systems closely is essential to mitigating this risk. Recommendations for preventive responses are considered across all levels of the justice system. The report includes a literature review that identifies a substantial body of research from various countries; this research supports the finding that the initial post-release period presents ex-prisoners with a significantly heightened risk of acute drug-related mortality.
